This is a Microsoft Word .docx template with sample provisions for parties to agree to the appointment of a Parenting Coordinator per the requirements of Probate and Family Court Standing Order 1-17.  For the agreement to be enforceable by the court it must be signed in writing by the parties and the parenting coordinator and incorporated into a Judgment.  We suggest filing such an agreement as a separate addendum from other parenting agreements.

This template contains only the sections required in order to outline the rights and obligations of each parent and the parenting coordinator related to the appointment.  All of these provisions may be modified to fit the unique circumstances of any particular family. We have included instructions throughout the template to assist with completing it.
